Documents Required to apply Work Permit

• Scan copy of valid Passport (All pages with marked)

• Complete Bio-data / CV / Resume with Permanent Address

• Visa application form: to be filled out and signed by the applicant

• Valid Passport continues to be valid at least six months after the intended departure from the Czech Republic / Schengen Area

• 2 color photo (35x45 mm) with white background.

• Employment permit issued by the Labor Office of the Czech Republic

• Accommodation Agreement

• Financial means – employer´s salary/wage confirmation – can be submitted in a form of an employment contract

• Verifiable evidence of sufficient means of subsistence during the intended stay: bank account statements for the last 3 months,

• Travel health insurance with the minimum coverage of EUR 30 000. The insurance must be valid for entire period of stay

• Cover letter: explaining the purpose of the trip

• Proof of return – Copy of round-trip flight ticket or flight reservation with valid reservation code or with a number of tickets, if already paid
